Our office staff works hard to ensure patient comfort and convenience. To help you better understand our office operations, we have included the information you need about our hours, locations, appointments, insurance acceptance and billing below.
***Please sign up now for the Patient Portal to be able to receive any required forms electronically. We expect to go paperless in the future, with all patients receiving all forms thru the portal system at no charge.****
The Medent Patient Portal provides you with on-line access for routine communication with Forestream Pediatrics through a secure internet connection. Please ask us how to sign up! You or your health insurer MAY be billed for portal questions that require significant or complicated responses from a provider.

Appointment times start 30 minutes after the phone hours begin and may extend up to 45 minutes beyond the end of the phone hours if needed to accommodate urgent care visits.
For urgent, life-threatening emergencies, call 911.
To speak with a provider on call, call the regular office number (716) 668-5331 and choose to speak with the answering service. They will contact the provider on call to return your call.
Non-urgent messages for the next business day can be left on the machine when the prompt "leave a message" is chosen.
We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly. We do require 24-hour notice of appointment cancellation. If less than 24 hours notice is given, a $25 cancellation fee will be assessed to your account.
Insurance and Billing
Please call or for information specific to your insurance company and plan. We accept most insurances.
We accept checks, cash or credit cards. Co-pays are due at the time of service. Any other balances are due upon receipt of the billing statement. We realize that there may be financial difficulties; our billing service (716) 639-0155, has a financial hardship and payment plans available.
