Insurances Accepted:

We participate with most insurance companies. It is your responsibility to be aware of your insurance coverage. We will bill your insurance, however, co-payments, denials and non-payment by an insurance company will be the patient’s responsibility. It is the responsibility of the parent that is bringing the child for the visit to pay the co-pay AT THE TIME OF SERVICE.

Payment Policy:

All Co-payments and deductibles are expected at the time of service.  Deductible plans require a $50 payment at the time of service until the deductible has been met or a FSA/HRA is available to cover the patient's deductible.  Payment is required at the time services are rendered.  This practice accepts cash, personal check, Visa, MasterCard, and Discover.  There is a service charge of $10.00 for payments that are not received on the date of service and a service charge of $20.00 for returned checks.  (Service charges are subject to change without notice)  If you have a past due balance, any amount paid will be applied to past due balance first.  Personal balances are payable upon receipt of statement.  If a personal balance is not paid within 30 days, a service charge of $10 will be added to the account.  

Patients with an outstanding balance  will be considered for release from the practice.  These accounts will be turned over to a collection agency unless prior arrangements are made.  We realize that there may be financial difficulties; payment plans are available by calling our Administrative Assistant at 716-668-5331.

Medical care is available for a sick child regardless of payment status.

Medical Advice:

Nurses are available to assist you with medical advice as recommended by our providers during regular business hours. After the office is closed, a provider is always on call for emergency concerns. If you experience a life-threatening emergency, please immediately call 911.

Test Results:

We will contact you with the results as they become available.  If you would like to call to confirm the results, please feel free to call during phone hours.

Prescription refills:

Please call our office during regular phone hours for your refill requests. Please allow 48 hours on all requests.  Most medications can be forwarded to your pharmacy electronically. Medications that are prescribed for ADD/ADHD may be e-prescribed if your pharmacy can receive controlled substances electronically, therefore, you may no longer be required to pick up the prescription at the office.

Referrals:

Requests for a referral to a specialist are reviewed by a Pediatrician. It may be necessary for a patient to be seen at our office prior to a referral. A referral appointment may be scheduled with our receptionists.

Sports/Camp/School Forms:

To protect a patient’s confidentiality, our office policy allows completed paperwork to be given, sent via portal, mailed if a self addressed stamped envelope is provided or faxed to the patient or parent/guardian rather than a third party such as a school, day care or employer.  Please allow one week for the completion of any forms.

Appointment times start 30 minutes after the phone hours begin and may extend up to 45 minutes beyond the end of the phone hours if needed.
