Forestream Pediatrics achieved the National Committee for Quality Assurance Level 3 Recognition for Patient Centered Medical Home for the fifth consecutive time beginning in 2013. Forestream Pediatrics has been serving the WNY community since 2003. We proudly serve as the school physicians for the Cheektowaga/Sloan Union Free School District, the West Seneca Central School District and the Lancaster Central Schools District. It is our goal to provide the highest quality medical care and service. In partnership with the patient and family, we are committed to providing preventative medicine and urgent care to improve the health of our community.
Wendy Gellman, MD
Wendy Gellman, MD graduated with a BS degree with Distinction from Cornell University College of Agriculture and
Life Sciences. She graduated magna cum laude from SUNY at Buffalo School of Medicine and Biomedical
Sciences. She completed her Pediatric Residency at Women & Children’s Hospital of Buffalo and is Board Certified
in Pediatrics.
Kimberly Prise, MD
Kimberly Prise, MD graduated cum laude from the University of Rochester with a BS in Molecular Genetics and from
SUNY at Buffalo School of Medicine and Biomedical Sciences with an MD. She completed her residency in Pediatrics
at Women & Children’s Hospital of Buffalo and is Board Certified in Pediatrics.
Jennifer Yunker, MD
Jennifer Yunker, MD graduated from Cornell University College of Agriculture and Life Sciences with a BS in Biology.
She graduated from Medical School at SUNY Stony Brook School of Medicine. She completed her Pediatric Residency at
Women & Children's Hospital of Buffalo and is Board Certified in Pediatrics.
Desiree Rebmann, FNP
Desiree Rebmann, FNP graduated from D'Youville College with a Bachelor of Science in Nursing and subsequently a
Master of Science in Family Nurse Practitioner in 2016. She is on the board of the National Association of Pediatric
Nurse Practitioners. Desiree started at Forestream Pediatrics in May of 2016.
Samantha Hurysz, PNP
Samantha Hurysz, PNP graduated from Case Western Reserve University with a Bachelor of Science in
Nursing. She graduated from Duke University with a Master of Science in Pediatric Nurse Practitioner.
She was inducted into the Sigma Theta Tau International Honor Society of Nursing.
Cassandra Swan, PNP
Cassandra Swan, PNP is a Board Certified Pediatric Nurse Practitioner. She earned her
nursing degree from St. John Fisher College and her nurse practitioner degree from
Stony Brook University. She is a member of the National Association of Pediatric Nurse Practitioners.
Chelsea George, FNP
Chelsea George, FNP is a board-certified Family Nurse Practitioner who completed her Masters
at D’Youville College in 2019. Previously Chelsea received her nursing degree
at St. John Fisher College graduating summa cum laude. She has worked with the pediatric population
for over ten years. Chelsea enjoys spending time with her family and friends, traveling, camping and hiking.
